  • completed Vanuatu passport replacement form;
  • 2 recent passport photos, taken within the last 6 months, with an identity witness signing and dating the back of one photo;
  • previous Vanuatu passport, if available;
  • police report or an immigration lost or damage declaration form for lost or damaged passports;
  • citizenship certificate;
