The exact document package can depend on the applicant’s situation and the reason for replacement. Based on official guidance for lost, damaged, and expired passports, applicants generally need the following:
- completed Vanuatu passport replacement form;
- 2 recent passport photos, taken within the last 6 months, with an identity witness signing and dating the back of one photo;
- previous Vanuatu passport, if available;
- police report or an immigration lost or damage declaration form for lost or damaged passports;
- citizenship certificate;
- copy of the birth certificate.
If the passport is being replaced after a change of name, the application should include details of all names previously used and supporting evidence of the change.
