If a Saint Lucia passport has been lost or stolen, the applicant must submit a witnessed statement as part of the replacement procedure. The statement must be completed in the prescribed form and certified with the signature and wet seal of a notary, lawyer or other authorised person.
In addition, the applicant must provide a copy of the police report. The report must be issued by the competent internal affairs authority in the country where the loss occurred and must be in English or accompanied by an official English translation.
